Cornwall House - Community Mental Health Centre

Community Mental Health centre.

The following services are provided:

Referrals Management Service (RMS)

This team is your first port of call for any referral into Cornwall House, alongside the CAS team. They will ensure all relevant information is collated and facilitate your referral through to either an appropriate community agency or health provider, or the appropriate team based within Cornwall House.

Community Acute Service (CAS)

This team provides a 24 hour, 7 day a week crisis assessment and treatment, and acute follow up in the community. They can also be contacted on (0800) 800 717.

Care and Recovery Team

Clients receiving continuing treatment will have their care coordinated by a key worker, who is a member of the multi-disciplinary team. The key worker takes responsibility for coordinating the treatment and care each person needs. Clients are able to access a range of more specialist services via their key worker, for example: CBT, DBT, dual diagnosis, family support, and a range of therapeutic groups or individual interventions that are available at the time.

Early Intervention

An intensive service for adults aged 18 - 30 who experience a first episode of psychosis.

Trans-cultural service

Clinicians with expertise in trans-cultural issues for clients from refugee or migrant backgrounds. This is linked to the Asian Mental Health service.

Service Coordination

Assisting clients to access a range of community based support services, including mental health residential rehabilitation services. This team is accessed via your key worker.
